San Francisco strictly requires the Assessor's Parcel Number (APN) and the situs (street address) on the face of the first page, along with the name and return address of the requesting party in the upper-left corner, a 3″×5″ blank area in the upper right for the recorder label, 8.5″×11″ paper, and the legal description of the property.
Deeds must be accompanied by a Preliminary Change of Ownership Report (PCOR) and a Transfer Tax Affidavit. Missing any of these triggers rejection — which is exactly what we prevent.
